Ideas for keeping flowchart and text description in sync?

Does anyone have any suggestions for creating a number of swimlane flowcharts, while maintaining a textual description of the processes at the same time?

I must create a set of flowcharts for a document. I must also include a step-by-step description of each process. I'm looking for a way to streamline the process, while ensuring the flowcharts and text are kept in sync.

Any ideas?
