Save (As) icons missing

Firstly, my complements to the team. This is a really user-friendly program. Well done.

My problem details:

When I go to File -> Save As, there are no icons on the buttons in the top right of the dialog box, e.g. "Up One Level", "Create New Folder", "List" and Details".

The buttons are there, the tool-tips work and they function correctly, but the icons are missing.

Similarly, File -> Export and File -> Open.
